Cancer Care Patient Care Navigator Position

This Full-Time position for either an RN or LPN is responsible to serve as an advocate for radiation and medical oncology patients serving as their primary point of contact and is responsible for coordinating the care and services of the identified patient populations for an extended period of time across the continuum of care both inpatient and outpatient services, promoting effective resource utilization, working with the multidisciplinary team to achieve an optimal clinical outcome. This position works closely with other healthcare disciplines to ensure timely appointments, results reporting, patient communication, care and follow-up. This position also interfaces with other healthcare teams for appropriate referrals/services (Social Services, Pastoral Care, Care Management, Community Services, Outpatient Services, Dietician, etc). This position requires an RN or LPN license, must be licensed in Montana or eligible to obtain a license. If an RN this position is represented by the Montana Nurses Association.
